Patient Appointments
- In-person appointments for all patients described in previous phase
- Prioritization of in-person follow up appointments for patients whose first appointment was telemedicine
- Prioritization of in-person appointments for select first time patients with special needs
- All other appointments via telemedicine
- Any patient may opt for telemedicine
Patient Requirements
- All patient require pre-approval by DCIM to come into the office
- Patients must self-certify the following:
- Patient is not at high risk for COVID-19
- No signs or symptoms of COVID-19 (e.g. respiratory symptoms, fever, sore throat, etc) in the last 14 days
- Patient has been practicing social distancing and not had contact with a confirmed COVID-19 case in the last 14 days
- If a patient is unsure, please contact us and we will help you determine your eligibility
- Patients must agree to
- Wear a mask the entire time
- Arrive on time, but do not enter the building more than 5 minutes prior to your appointment
- Wash or sanitize your hands immediately upon entering the floor
DCIM Precautions
- Daily site wide deep clean and disinfection, including all practitioners on our floor
- Inter-appointment disinfection of offices
- Monitoring of all practitioners to ensure continuous 14-day symptom free certification and 14-day quarantine for practitioners with potential exposure
- Reduced capacity to no more than 25% of maximum for entire floor to provide for easy social distancing
- Daily site wide deep clean and disinfection, including all practitioners on our floor
- Inter-appointment disinfection of offices
- Monitoring of all practitioners to ensure continuous 14-day symptom free certification and 14-day quarantine for practitioners with potential exposure
- Reduced capacity to no more than 25% of maximum for entire floor to provide for easy social distancing
- Minimum-contact sanitation stations for patients
